There has been widespread impact on communities throughout the Northern Grampians Shire due to the October 2022 floods. If you have been affected, please let us know either by completing this form or calling the Northern Grampians Shire Council Flood Recovery Hotline on 03 5358 9099 from 9am-4.30pm daily Monday-Friday (excluding Public Holidays).

The form will take between five and ten minutes to complete and the information collected may:

• Be used by the council and/or recovery service providers to provide you with information about, and access to, relevant recovery support services and programs.
• Be used by council and/or recovery service providers to plan, coordinate and deliver recovery support services and programs for the October 2022 flood event.
• Be shared with Commonwealth Government and municipal council agencies and organisations, and not-for-profit and non-government organisations for the purposes outlined above.
• Be aggregated and/or de-identified and used by the council to report on and evaluate the progress and outcomes of recovery programs and operations.
